Multiple dates availableUpon arrival, please inform your server or manager of your chosen Party Package. To ensure a smooth experience, at least one member of your party should arrive within 15 minutes of your reservation. Late arrivals may result in releasing the table. No refunds will be given. You may bring freestanding or floating decorations, but confetti, glitter, and similar items are prohibited and may incur cleanup fees. Pricing excludes tax and gratuity. Per Maricopa County Health Codes, outside food and drinks are prohibited. A $100 deposit is required to secure your reservation, non-refundable and applied to your final bill. Deposits are non-refundable. The Forum may cancel reservations for event buyouts, with full deposit refunds processed.
